Job Description:

Communicates with patients and families after discharge to address concerns and ensure understanding of follow-up care and functional assessment.
Collaborates with the multidisciplinary team to ensure high-quality patient care.
Participates in community education as needed.
Maintains professional relationships with all customers and completes assigned work comprehensively and in a timely manner.
