Physical Requirements: Satisfactory physical examination as determined by the Employee Health Unit upon appointment only. The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. The VA Medical Center is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity (glasses permitted), keen hearing (hearing aid permitted), clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking (up to 8 hours), standing (up to 8 hours), stooping, sitting, bending (up to 8 hours), pulling, and pushing. Light lifting under 15 lbs. Light carrying 15-44 lbs. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. Use of rapid mental and muscular coordination simultaneously. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. Develop tracking mechanism for high risk women; and track and follow up on this population Perform risk calculation on women Veterans and provide this information and recommendations to the WHC and PC providers. Assist the WHC and PC PACT teams with coordinating subsequent care for women with high risk scores, and with patient communication and navigation Coordinate care with WHC and PC providers, cardiology pharmacists and cardiology Collaborate with dWHPs, PCPs, Pharmacy, Nutrition Services, Physiatrist & Medicine, and Cardiology Identify knowledge deficits related to heart health screening and risk reduction and provide education to improve competencies for WH Liaisons and PACT RNs while integrating Whole Health principles. Provide heart health screening education and training to PACT RNs Assist with gender disparity data analysis and action plan development and implementation as the Women's Health Program's gender disparity champion Utilize available and emerging technologies and tools to communicate with and provide behavioral modification strategies and reminders (e.g. Annie, VA Video Connect, MyHealtheVet, etc.) Other Duties Provide 2nd line back-up for Maternity & Specialty Care Coordinator and Breast Cancer Screening Coordinator as needed Provide back-up for WH specialty RNs and WHC-PACT RNs needed Provide coverage of EOC rounds and nursing orientation for WVPM as needed Work Schedule: Standard options are Monday through Friday, 7:30 a.m. to 4:00 p.m. or 8:00 a.m. to 4:30 pm, but other options may be needed or negotiable.
